Without a time we use noon. The Moon moves about 13° a day, so the nakshatra or pada can change on that day.
How it works कैसे काम करता है
We compute the Moon's true position for your birth moment, subtract the Lahiri ayanamsa to get its sidereal longitude, then read the rashi (30° each), the nakshatra (13°20′ each) and its pada (3°20′ each). With a birth time we also find the rising sign (lagna).

Usually yes. The Moon stays about 2.25 days in one sign, so date and place are enough most of the time. Nakshatra pada and lagna need the time.
